You aren’t reaching out to connect with everyone that you've worked with, has worked for you, you’ve worked for or had a working relationship with.
Take some time to make a list of everyone that you can think of who would be great to reconnect with. Do a search, find them and asked to become a connection.

You aren’t hanging out in the right groups. It's not a bad idea to join up with groups that are within an industry, market segment or topic that you're interested in becoming involved with. Take a look at all of the people that are members and reach out to those that you believe you may have some synergies with.
You aren't following up or interacting with people. If you don't make time to connect, contact, comment and mingle, you are missing out on one of the most powerful aspects of LinkedIn ... human interaction and relationship building.
You don't check in comment or connect regularly. It always floors me that people have the misconception that placing their resume online is going to drive tons of leads, connections, and opportunities directly into their inbox. This is not at all accurate. You have to get out there and contribute in order to receive.
LinkedIn is a phenomenally powerful tool and network which you can access from anywhere in the world. It gives you the ability to reach out to virtually anyone you would like to come in contact with for business purposes, but it is on you to create a presence and establish a voice for yourself in order to build relationships.
So get out there and make it happen. Your future self will thank you!
